Patient Advocacy and Liaison services at MedCentre are designed for individuals and families who feel lost in a maze of appointments, diagnoses, and medication lists. As experienced clinical pharmacists with a focus on mental health and chronic illness, we stand beside you to help organize information, ask the right questions, and keep your care team aligned.
Many of our clients see several providers-primary care, psychiatry, therapy, cardiology, pain management-and are unsure who is watching the whole picture. Through Interdisciplinary Communication Facilitation, we coordinate with your clinicians, sharing clear, clinically grounded medication recommendations and ensuring that each provider understands what the others are prescribing and why.
We begin by reviewing your history, medication regimen, and current concerns. From there, we help you prepare for key visits by identifying priority questions, clarifying your goals, and translating medical language into terms that make sense. With your permission, we can also send concise summaries or consult notes to your care team, reducing the chance of conflicting instructions or duplicated therapies.
For seniors and caregivers, this advocacy can mean fewer unnecessary drugs, better monitoring for side effects like falls or confusion, and more confidence in speaking up when something feels wrong. For adults with complex mental health needs, it can bring structure and safety to treatment plans that involve multiple psychotropic medications and frequent adjustments.
